	Ukraine Visa Application (fill-in) NOTE: EFFECTIVE JULY 1, 2005 USA CITIZENS TRAVELING TO UKRAINE FOR A PERIOD OF 90 DAYS OR LESS ARE NOT REQUIRED TO HAVE A VISA. The visa application MUST be completed according to some very specific guidelines. This is your permission to enter Ukraine. Problems in obtaining a visa in a timely manner can lead to disqualification from your team. Therefore it is critical that you review the additional information and that your work closely with your Team Coordinator. More info
